Telemedicine FSD diagnostics in the medical examination system

In September-November 2017, a pilot project "Evaluation of the effectiveness of the use of FSD-telediagnostics in the medical examination system" was carried out, which was conducted by order of the Chairman of the Health Committee of the Minsk City Executive Committee, S.S. Malyshko, based on a letter from the Doctor of Medical Sciences, director of the Republican Scientific and Practical Center for Medical Technologies, Informatization, Management and Economics of Healthcare M.M. Sachek dated 12.06.2017 No. 01-05/427.

The project was carried out on the basis of the UZ "35th City Clinical Polyclinic" of Minsk together with the Zdrava medical center of Profdiag LLC, Spectral-Dynamic Systems LLC and Delovaya Net LLC. The technical specification for the project was approved by the Chief Physician of the UZ "35th City Clinical Polyclinic" of Minsk, A.N. Linderov.

Project objectives:

The main objectives of the pilot project were:

  • Confirmation of the reliability of medical diagnostic information obtained with the help of FSD-express diagnostics.

  • Evaluation of the technological effectiveness of FSD-express diagnostics.

  • Evaluation of the economic efficiency of FSD-express diagnostics.

  • Evaluation of the prospects for the use of the technology of FSD-express diagnostics in the system of medical examination of the population.

Project progress:

The implementation of the project was based on the methods of express diagnostics of nosological risks, early (latent) stages and manifest forms of diseases based on FSD diagnostics using a medical device, the medical spectral-dynamic Complex (CMSD).

The technological process within the framework of the Technical Task dated 09/20/2017 for the pilot project included recording the patient's FSD signal on KMSD-1 (located in the polyclinic), transmitting data to KMSD-2 (located in the Zdrava medical center) via the Internet, conducting remote (telemedicine) FSD-express diagnostics by a doctor, compiling the diagnostic report and the transfer of the diagnostic report to the polyclinic.

The operation of the remote FSD-express diagnostics system in the medical examination system in the industrial mode is supposed to be carried out on the basis of the network version of the СMSD, which includes a nursing terminal, secure communication channels, a server and a medical terminal. Within the framework of the CMSD network version, the technological process includes recording the patient's FSD signal at the nursing terminal, transmitting recorded data and passport information via a secure communication channel to the server, processing data on the server, transmitting data to the medical terminal, conducting an FSD express diagnosis by a doctor and drawing up a diagnostic conclusion, transmitting the resulting conclusion to the server and further to MIS polyclinics for the district therapist.

57 patients participated in the project. According to the Technical task of the FSD-express diagnostics was carried out on 6 body systems:

  • digestive system;

  • cardiovascular system;

  • genitourinary system;

  • nervous system;

  • bone and joint system;

  • bronchopulmonary system.

In total, the diagnosis of 57 patients in 6 body systems amounted to 342 diagnostic cases. Examples of individual patient protocols have shown that FSD-express diagnostics provides identification of functional processes characteristic of the development of diseases, as well as for the stage of actual individual risk of diseases.

The report provides examples of matching diagnoses.

162 diagnostic cases were subject to comparative analysis (with the exception of 180 cases that are not subject to diagnosis, according to the Terms of Reference regarding the implementation of the initial and boundary conditions of FSD diagnostics)

Of the 162 diagnostic cases, the compliance of clinical diagnoses with the ultrasound of the 35th HCP and the results of FSD-express diagnostics was established in 151 diagnostic cases, which is 93.2%.

Thus, the reliability of FSD-express diagnostics was more than 93%, and the error rate did not exceed 7%, which is a very good result for any express diagnostics.

In addition to the analysis of diagnostic correspondences, of great interest is the analysis of the structure of diseases (including their actual individual risks) that were first identified in the examined patients in the process of FSD-express diagnostics.

Note that the call center for remote FSD diagnostics can perform not only FSD express diagnostics, but also in-depth FSD diagnostics. This gives additional opportunities in the organization of the medical examination process at the post-screening stage, that is, at the stage of dispensary observation.

As a result, the technology of telemedicine FSD diagnostics provides early diagnosis of diseases already at the screening stage of medical examination of the population and provides the advantage of the proposed system of medical examination of the population.

The results showed that:

  • FSD-express diagnostics provides early diagnosis of pathological processes in the main body systems;

  • The medical diagnostic information obtained with the help of FSD-express diagnostics is reliable (the correspondence of diagnoses is more than 93%);

  • Nosological FSD-express diagnostics at the screening stage of the medical examination system has high technological efficiency;

  • Nosological FSD-express diagnostics at the screening stage of the medical examination system of the population has high economic efficiency;

  • The prospect of using FSD diagnostics is to create a diagnostic telemedicine system operating in the mode of autonomous data collection and automatic processing;

  • FSD-express diagnostics can be recommended during the medical examination of the population.
